Immunizations

When you register your child for school, it will be necessary to provide a copy of his/her complete immunization record to the child's school.  It is important that any needed immunizations be obtained prior to the day of registration.  If you have any questions, please consult the school nurse at any of our schools.  

Every child entering the Early Childhood Program for 4 year olds must have received all of the vaccines, be in-process for receiving needed doses or show proof that they have applied for an exemption for these vaccines: Diphtheria, Tetanus, Pertussis, Poliomyelitis, Haemophiles Influenza Type B,  Hepatitis, Red Measles (Rubeola), Rubella,  and Varicella (Chickenpox).

All children attending school in Pre-Kindergarten through grade twelve must show proof of immunization, be in process for receiving needed doses or show proof of exemption against: Poliomyelitis, Diphtheria, Tetanus, Pertussis, Red (Rubeola) Measles, Rubella, Mumps, Hepatitis B, and Varicella (Chickenpox), as evidenced by a certificate of a licensed physician or a public health department acknowledging the immunization. 

STUDENTS WILL NOT BE ALLOWED TO ATTEND SCHOOL WITHOUT HAVING ALL REQUIRED IMMUNIZATIONS

Physical Examinations

All students who are entering a LRSD school for the first time are required to have a physical examination by their doctor or primary health care provider.  Students transferring from another district must provide a copy of a physical exam in their transfer file or obtain a new physical.  If you have any questions about this physical, consult with the school nurse at any of our schools.

Please schedule your child's pre-school physical in the spring and request a copy of the physical to provide to the school.  Do not wait until August when appointments are not available.

Individual Healthcare Plans (IHPs) and Action Plans

Individual Healthcare Plans or Action Plans are required for all students with on-going health needs (Asthma, Diabetes, Food Allergies, Seizures, etc.)  

Forms for these requirements are available at school and on the district web page, www.lrsd.org.  On the District's website, click on the department tab, then the Health Services link.  Next, click on Health Services Home Page, and finally Forms - which appears in the right column.  

NOTE:  LRSD accepts any physician's forms. You do not have to use LRSD forms to meet state laws.The physician exam must document all systems assessed including height, weight, hearing and vision.
The form must be signed by your child's physician, school nurse and principal before the child starts his/her first day of school.

School Nurses

Professional nurses are assigned to every school and are available to all students.  A nurse may be in the school only one day per week.  Nurses provide care for sick and injured students, screening and assessments, health counseling and other health related services.  Parents are encouraged to discuss the health needs of their children with the school nurse, teacher, and principal.

Medication Policy

Students are provided the opportunity to take medications that have been prescribed by a physician and provided by the parent / guardian.  Written permission from the parent / guardian must be received before the medication may be administered by school personnel.  Parents / guardians must bring the medication to the school.  Students may not transport medication to or from school.  All unused medication must be picked-up by a parent or guardian.  Students who have recently had surgery or a medical procedure and need prescribed pain medication should not attend school.

School nurses need to know all medications that students are taking, even those given at home, to assist parents in watching for benefits and side effects.
