Children who are 3 or 4 on or before August 1, 2024, are eligible to apply.

(child's birth certificate, proof of current address, and last 30 days of income)

FIRST DAY OF SCHOOL INFORMATION:
Print the Pre-K Physical Form (well-child health form) and have the pediatrician complete it. Submit the physical and up-to-date immunizations to the school nurse on the first day of school.  

Pre-K Physical Form (3 and 4-year-olds)

The Early Childhood Office is located within the Geyer Springs Early Childhood Center, 5240 Mabelvale Pike (come to the entrance off 51st Street).

Office hours are 8:00 am-4:30 pm Monday through Friday.  We assist families onsite between 8:00 am-3:00 pm, Monday through Friday. We are available by phone during office hours.
